We are seeking a compassionate and detail-oriented Patient Care Coordinator to support administrative functions within our healthcare practice. This role serves as a key point of contact for patients, ensuring a smooth, efficient, and positive care experience from check-in through follow-up. The ideal candidate is patient-focused, highly organized, and comfortable balancing hands-on clinical duties with care coordination responsibilities.

This position will primarily be based at our Tarzana office.

Key Responsibilities

  • Serve as the primary point of contact for patients, coordinating appointments, referrals, and follow-up care
  • Support patient intake, scheduling, and documentation within the EHR system
  • Educate patients on procedures, care plans, and next steps to ensure understanding and compliance
  • Coordinate diagnostic testing, lab work, and external referrals
  • Maintain accurate and timely patient records in compliance with HIPAA and clinical standards
  • Assist with prior authorizations, insurance verification, and patient communication as needed
  • Collaborate closely with providers, nurses, and administrative staff to optimize patient flow

Qualifications

  • Certified Medical Assistant
  • 2+ years of experience in a clinical healthcare setting preferred
  • Strong patient communication and interpersonal skills
  • Proficiency with EHR systems and basic medical documentation
  • Ability to multitask and stay organized in a fast-paced environment
  • Knowledge of HIPAA, infection control, and clinical safety standards
