Job Summary

Creative Home Health is seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing services and complete OASIS assessments in the home setting.

Responsibilities

• Complete Start of Care (SOC), Recertification, and Discharge OASIS assessments
• Provide skilled nursing care per physician orders
• Accurately document in the EMR in compliance with CMS and agency standards
• Coordinate care with physicians and interdisciplinary team members
• Monitor patient progress and report changes in condition

Requirements

• Active Florida RN license in good standing
• Minimum 1 year of recent Home Health experience
• Hands-on OASIS assessment experience (SOC/ROC/Recertification) is a plus
• Current CPR/BLS certification
• Ability to work independently in the field
• Reliable transportation
• Level II background screening required
