The Telephonic Nurse Case Manager (TCM) is a Licensed Practical Nurse (LPN) who provides case management services to individuals who are injured in the workplace.
This position can be performed only by an individual with a current, unrestricted LPN who is licensed to practice in Pennsylvania
This position will be performed entirely from our Mechanicsburg, PA office, located at 1970 Technology Parkway, Mechanicsburg, PA 17050.

The TCM nurse assesses and documents treatment and recovery activities to facilitate the timely return to work of individuals who have been injured in the workplace. The TCM nurse reviews all medical aspects of workplace injury claims, evaluates and/or recommends diagnostic procedures and testing, and coordinates second opinion examinations. The TCM nurse serves as a communication conduit and resource among insurance carriers, medical providers, insureds/employers, and injured workers, to ensure the success of the treatment and recovery plan. The TCM nurse evaluates treatment progress and establishes and refines rehabilitation goals and outcomes. Specific duties include:
• Establish telephone contact with the injured worker to conduct intake interview and obtain injury, treatment and other current information
• Coordinate medical care by developing, initiating and instituting a rehabilitation plan for the injured worker
• Determine if the injured worker is treating within the requirements of prevailing statutes
• Work cooperatively with other rehabilitation professionals to ensure appropriate and adequate physical care is being provided
• Maintain contact with treating physician and other care providers for updates and ensure injured worker compliance with all medical treatment
• Contact employer to discuss injured worker's condition, determine employment availability, and ascertain relevant duties and essential functions of pre-injury and/or alternate job(s)
• Contact injured worker and physician's office after each medical appointment to discuss progress and ongoing treatment plan(s)
• When injured worker obtains a release to return to work, contact employer and facilitate successful return to work
• Monitor and ensure the success of the return to work through regular contact with worker and employer
• Complete and submit various reports for insurance carrier/self-insured throughout the care management process
• Refer cases to field-based medical management, as necessary and approved
• Act as liaison, coordinating and communicating efforts among all stakeholders - including the injured individual, physicians and health care providers, attorneys, employers and insurance professionals.
MINIMUM QUALIFICATIONS
IF YOU ARE NOT AN LPN LICENSED/CERTIFIED TO PRACTICE IN PENNSYLVANIA, OR ARE UNABLE TO WORK FROM OUR MECHANICSBURG, PA OFFICE, PLEASE DO NOT APPLY FOR THIS POSITION:
• LPN with a current and unrestricted PA license
• At least 2 years of applied nursing experience on a hospital or triage level
