Job Title: MSW Social Worker
Position Information
- Start Date: May 12, 2025
- Schedule: Monday – Friday, 8:30 AM – 4:30 PM (Weekends as needed)
- Work Location: Pompton Plains, NJ 07444
- This is a 13 week contract with potential to extend
Job Summary
The Care Coordination MSW Social Worker collaborates with a multidisciplinary team to provide patient-centered, outcome-based care. This role supports patients and families by addressing social determinants of health (SDOH), ensuring safe transitions across the care continuum. We are looking for 2 LMSW's to join our team!
Key Responsibilities & Time Allocation
- 60% – Patient assessment, referrals, interventions
- 20% – Documentation
- 10% – Meetings and program development
- 10% – Collaboration with internal/external agencies
Detailed Job Functions
- Support high-risk/high-cost patients by addressing barriers to care and connecting them to sustainable medical and social resources.
- Conduct psychosocial assessments and SDOH screenings; develop individualized care plans based on patient goals and needs.
- Provide telephonic or virtual counseling using evidence-based interventions to support wellness and self-management.
- Ensure consistent communication with the care coordination team, providers, and community resources.
- Assist with navigation of medical, social, insurance, and benefit systems; maintain updated community resource knowledge.
- Participate in department meetings, trainings, and provide consultative support to community health workers.
- Perform other duties as required by leadership.
Required Skills:
- 3–5 years of experience in a health-related field; hospital experience preferred
- Case Management Experience is required!
- Strong understanding of social services, community resources, and support systems
- Familiarity with Epic EMR preferred
- Proficiency in Microsoft Office and EMR documentation standards
- Excellent organization, communication, and interpersonal skills
- Ability to work collaboratively with interdisciplinary teams
- Compassionate and culturally competent service delivery
- Bilingual skills a plus
Required Education & Licensure
- Education: Master's Degree in Social Work from an accredited school
- Licensure: Active LSW license in the State of New Jersey (in good standing)
- Preferred Background: Experience in Acute Inpatient Case Management and Discharge planning
