Physical Therapist (PT) – Assisted Living Facility | Texas
Salary Range: $95,000 – $120,000 per year
Location: Midland, Texas
Job Type: Full-Time
Schedule: Monday to Friday, 9 AM – 5 PM
We are a leading provider of therapy services in senior living communities throughout Texas. Our mission is to help older adults maintain independence, improve mobility, and enhance quality of life through individualized, compassionate care. Our team works closely with residents and staff in a supportive, resident-focused environment.
We are seeking a Physical Therapist to deliver outpatient-style rehab services directly within Assisted Living Facilities. This role is ideal for clinicians who are passionate about promoting aging in place and functional independence in a residential setting. You’ll work autonomously but collaboratively within each community to support resident wellness and recovery.
Key Responsibilities:
- Evaluate residents and develop customized treatment plans
- Provide one-on-one therapy to address mobility, balance, fall prevention, and functional goals
- Reassess progress and adjust therapy plans as needed
- Document treatment sessions and maintain compliance with Medicare and facility guidelines
- Educate residents, families, and staff on exercises, safety strategies, and wellness
- Collaborate with facility staff, nursing teams, and family members
Requirements
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program
- Active Physical Therapist license in the state of Texas
- Prior experience with geriatric patients or in assisted living is a plus
- Familiarity with Medicare Part B outpatient services
- Excellent interpersonal and communication skills
- Compassionate, proactive, and respectful of resident dignity and autonomy
