Physical Therapist

Reports To:

Director of Rehabilitation Services

Summary:

The incumbent shall be responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, state and federal guidelines, professional practices and ethical standards. The incumbent shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary fashion. The incumbent shall be accountable for carrying out all the responsibilities in accordance with Ave Maria’s values and mission.

Qualifications:

  • Graduate of a four-year bachelor’s program and hold an applicable and current license/certification to practice.
  • Experience working with a geriatric population is preferred.
  • Clinical skills to identify patient needs and develop a comprehensive plan of care to address those needs.
  • Demonstrate competency in using a computer or tablet for electronic medical records.
  • Good communication skills in order to interact with patients/residents, caregivers/family and other members of the IDT team.

Essential Clinical Functions:

  1. Complete screening on patients/residents as clinically necessary.
  2. Complete all clinical documentation in accordance with state and federal guidelines.
    1. Conduct initial evaluation, complete daily notes, recertification and discharge summary.
    2. Demonstrate effectiveness and impact of therapeutic intervention.
    3. Evaluate patient/resident response to treatment.
  3. Provide patient/resident and family/caregiver with education and training that assist with continued functional independence.
  4. Supervise PTA, PT/PTA students and rehab tech in accordance with state and federal guidelines.
  5. Participate in facility quality management activities.
    1. Participate in survey preparation activities.
    2. Participate in quality assurance programs.
  6. Coordinate with other members of the Interdisciplinary team (IDT).
    1. Participate in IDT care conference meetings as directed.
    2. Update IDT team and physician on patient/resident status.
    3. Attend other facility meetings as directed.
    4. Discuss with DOR any issues regarding patient care as soon as they arise.
  7. Participate in continuing education/professional development activities.
  8. Maintain license/certifications and memberships in applicable professional organizations.

Physical Requirements:

  1. Ability to travel in the community to meet patients/residents/customers.
  2. Frequent and prolonged bending, stooping, crouching, walking and standing.
  3. Ability to communicate and write in English.
  4. Ability to transfer patients and equipment (requirements may vary upon facility).
