Job DescriptionSalary: $50,000 - $55,000

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Works collaboratively with Quality Improvement (QI) Manager/Coordinator and Team to develop and implement Continuous Quality Improvement Plan.
  • Understand and implement policy and guidelines/standards related to Federally Qualified Health Centers, Patient Centered Medical Homes and Meaningful Use.
  • Analyze, interpret, and use data to collaborate, and identify best practices and areas for improvement.
  • Monitor and assists staff with improving documentation to support required data entry and reporting.
  • Conduct data analysis and research data sources to account for and make any corrections to existing data as needed to ensure accuracy of the information being recorded.
  • Extract and report data to meet local and federal program requirements, support agency objectives and internal management decision making processes.
  • Assist in the development of evaluation tools that will produce quantitative and qualitative data and findings to validate compliance and standards of care for services.
  • Conduct internal audits and chart reviews to assist with the monitoring and tracking of data quality to improve data output.
  • Monitor and measure indicator and outcome measures to evaluate effectiveness of applied standards.
  • Create and disseminate reports utilizing reporting tool.
  • Troubleshoot and respond to electronic health record (EHR) and QI support tickets.
  • Conduct patient surveys to gather patient feedback to inform QI processes.
  • Assist with EHR buildouts and templates to support reporting.
  • Facilitate EHR training for new and current staff.
  • Adhere to agency policy and procedures, maintain data integrity and abide by Patient Rights, and Health Insurance Portability and Accountability Act (HIPPA) Privacy Standards.
  • Attend relevant meetings to support overall program implementation and growth.
  • Attend trainings and learning opportunities that expand content knowledge and best practices.
  • Other duties as assigned



QUALIFICATIONS, KNOWLEDGE & SKILL REQUIRED

  • Bachelors Degree in Information Technology, Public Health, Social Work or other applicable field.
  • High degree of independence, flexibility, initiative and commitment
  • Strong analytic and problem solving and verbal and written communication skills.
  • Detail-oriented and can consistently meet deadlines and work well within a team environment.
  • Strong planning, organizational and critical thinking skills.
  • Ability to prioritize work and multi-task
  • Proficiency with Microsoft Word, Excel, and PowerPoint
  • Ability to work collaboratively with physicians, staff and external organizations




PREFERRED QUALIFICATIONS

  • Background with experience working in a health care setting
  • Experience in database/software development
  • Electronic Health Record (eClinicalWorks) and reporting tool (BridgeIT) experience.
  • 1-2 years experience with quality improvement and health care clinic operations
