\n\nQualifications for RN Case Manager & Social Worker Jobs\n

• Active Massachusetts RN license or Social Worker license (LICSW/LCSW) required\n
• Minimum 1 year of experience in acute discharge planning or case management\n
• Experience in an inpatient rehab hospital or neuro unit preferred\n
• Familiarity with Cerner EMR preferred (training provided on unique rehab platform)\n
• Strong organizational, communication, and clinical support skills\n
• Ability to manage a caseload of 16-20 patients (based on census)\n

\n\nKey Responsibilities\n

• Coordinate and implement comprehensive discharge planning for Neuro Unit patients\n
• Collaborate with interdisciplinary healthcare teams to ensure seamless transitions of care\n
• Communicate effectively with patients, families, and external healthcare providers\n
• Document all case management activities in the Cerner EMR system\n
• Advocate for patient needs and support optimal rehabilitation outcomes\n
