Most providers spend their careers treating problems after they become emergencies.
A wound worsens.
An infection develops.
A resident is sent to the emergency room.
A hospitalization occurs.
Then healthcare reacts.
At Twin Cities Physicians, we believe some of the most valuable providers are the ones who prevent those situations from happening in the first place.
We're seeking a full-time Wound Care Nurse Practitioner who enjoys identifying concerns early, intervening before complications occur, and helping patients remain healthier where they live.
Our providers deliver care directly inside assisted living communities, memory care settings, transitional care programs, and long-term care communities throughout the Twin Cities Metro. Rather than waiting for patients to navigate the healthcare system, you'll bring specialized wound care directly to them, helping improve outcomes while reducing avoidable hospitalizations and complications.
Organizations such as Bluestone Physician Services, Genevive, and Lifespark have helped demonstrate the value of community-based medicine. Twin Cities Physicians continues to expand throughout Minnesota by bringing proactive, specialized care directly into senior communities.
Why Providers Join Twin Cities Physicians
• $120,000 to $160,000+ total compensation opportunity through a combination of salary and productivity incentives.
• Become a key clinical resource helping patients avoid unnecessary setbacks, complications, and hospital admissions.
• Manage a wide variety of wound care cases while developing expertise in geriatric and post-acute medicine.
• Join a growing provider organization serving senior communities throughout Minnesota.
• Receive a comprehensive compensation package that includes mileage reimbursement, CME allowance, malpractice coverage, PTO, and full benefits.
• Practice within a collaborative physician-led team that values clinical judgment and proactive care.
What You'll Be Doing
• Provide advanced wound care assessments, treatment planning, and ongoing management within senior living communities.
• Conduct routine rounding visits and follow-up care across assigned communities.
• Manage medications, diagnostics, treatment plans, and clinical interventions.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Identify risks early and implement strategies that help prevent complications and hospitalizations.
• Support continuity of care while improving healing outcomes and quality of life.
Qualifications
• Active Minnesota Certified Nurse Practitioner (CNP) license.
• Experience in wound care, geriatrics, primary care, post-acute care, skilled nursing, or related settings preferred.
• Strong clinical judgment and confidence managing medically complex patients.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
The best healthcare outcomes often come from the problems patients never experience.
If you're looking for a full-time opportunity where your expertise helps prevent complications, improve healing, and keep patients healthier in the communities they call home, we'd love to connect.
