Compensation
- Gross Weekly Pay: $2,877.50
- Weekly Stipend: $2,240.00
- Weekly Taxable Wages: $637.50
- Local Pay Rate: $55–$61/Hour
Position Overview
We are seeking a Licensed Master Social Worker (LMSW) to provide psychosocial support and care coordination for patients in an acute care hospital setting. The LMSW will conduct assessments, provide supportive counseling, facilitate discharge planning, and coordinate community resources to support patient care transitions.
Required Qualifications
- Master's Degree in Social Work
- Active New York LMSW License (NYSED Primary Source Verified)
- Minimum 1 year of LMSW experience
- English proficiency required
Key Responsibilities
- Conduct psychosocial assessments for inpatient medical and surgical patients
- Provide supportive therapy and emotional support to patients and families
- Develop and coordinate discharge plans
- Facilitate referrals to community resources and support services
- Collaborate with interdisciplinary healthcare teams
- Assist with care transitions and continuity of care planning
- Maintain accurate and timely documentation
