As a Home Health Speech Language Pathologist at CenterWell Home Health, you’ll play a crucial role in helping individuals regain their strength, mobility, and independence—all from the comfort of their homes. Your Role and Responsibilities

As a vital member of our team, you will:

• Evaluate and Direct: Provide comprehensive speech/language pathology services to patients in their homes or facilities.

• Collaborate: Participate in developing and reviewing the Plan of Treatment and Plan of Care.

• Assess and Treat: Utilize your professional skills to identify and minimize the effects of speech, voice, language, hearing, and swallowing disorders.

• Administer Tests: Conduct diagnostic tests and apply therapeutic treatments, including audiologic screenings.

• Monitor Progress: Observe and report changes in patient conditions to supervisors and physicians.

• Educate: Provide training on alternative communication systems and counsel patients, families, and healthcare staff.

• Document: Maintain accurate documentation and submit timely reports as required.

• Coordinate Care: Engage in care coordination activities and discharge planning.

• Uphold Standards: Maintain confidentiality and the highest standards of professional conduct.

• Develop Professionally: Participate in internal staff development programs and continuing education.

What You Bring to the Table

We’re looking for candidates who meet the following qualifications:

• Certification of Clinical Competence in Speech Language Pathology or Audiology from ASHA.

• A minimum of six months of experience as a speech therapist/speech language pathologist.

• Home health experience is a plus!

• Current and unrestricted license and CPR certification.

• Strong organizational and communication skills.

• A valid driver’s license, auto insurance, and reliable transportation.

Compensation and Benefits

We value your contributions and offer a competitive pay range:

• Pay per Visit: $49.00 - $69.00

• Annual Base Pay: $77,200 - $106,200
