Assist Case Management in fulfilling duties of the department by helping to organize and coordinate office operations. Assist with procedures and resources to facilitate effectiveness and efficiency with all members of the Case Management Staff.
Main Responsibilities
- Serve as the primary responder for all incoming calls, ensuring prompt and efficient messaging to the appropriate staff members. All communication should be conducted in a timely, respectful, and courteous manner.
- Promptly respond to departmental email and distribute as necessary. Transfer ADRC voicemail email transcriptions to Excel log.
- Ensure department office supplies are tracked, distributed to outer county employees and reordered as necessary.
- Responsible for departmental mail including distributing, scanning, date-stamping, and postage.
- Keep all case management filing current.
- Import and export data from case managers and from state into case management database program.
- Print and mail Health and Wellness Medicaid Waiver letters received from the state.
- Assist with submission of CHOICE Medicaid applications.
- Cover front desk when necessary.
- Create reports, flyers, certificates and other materials as needed for projects.
- Coordinate and track home and vehicle modification projects from start to finish, ensuring timely execution and completion through all steps of the project (including billing). This includes providing continuous and thorough communication with providers, Care Manager and others as needed, throughout the process.
- Responsible for shopping for furniture and other necessities for Money Follows the Person (MFP) clients who are being discharged from nursing homes into their own homes. Coordinate deliveries.
- Tracking money spent through the MFP Community Transitions Fund, reconciling with fiscal, and making sure billing is done timely.
- Help with the coordination of TRIP tickets.
- Assist with Pooled Trust clients, as needed.
- Assist with annual client satisfaction surveys.
- Put together Intro packets for case managers.
- Manage and organize the emergency Food Pantry for our Care Manager.
- Co-ordinate Angel Tree program.
- Send out monthly schedule calendars to staff.
- Process monthly mileage sheets for case management staff and reservation logs for company vehicles.
- Update staff and vendor phone list and disaster plan contact list.

Qualifications
High school diploma. Knowledge of accounting, data and administrative management practices and procedures and basic business principles. Must be dependable and punctual. Strong skills in computer competency, time management and organization. Must be able to maintain an enthusiastic professional attitude within a stress-related atmosphere.
