Job Title: Occupational Therapist – Home Health (Contract)
Location - Tacoma, WA
Shift: 8-hour shifts
Guaranteed Hours: 40 hours/week
Contract Length: Contract assignment
Job Summary
We are seeking a licensed Occupational Therapist to provide coordinated, high-quality patient care in a home health setting. This role focuses on helping patients of all ages develop, regain, or maintain independence and improve their ability to perform daily activities.
The Occupational Therapist will evaluate functional performance, design and implement individualized plans of care, and collaborate with an interdisciplinary team to achieve optimal patient outcomes.
Key Responsibilities
Perform comprehensive clinical assessments and determine patient needs and appropriateness for occupational therapy services
Develop, document, revise, and complete individualized Plans of Care (POC), progress notes, and related clinical documentation
Provide direct therapeutic interventions to improve:
Range of motion, strength, and coordination
Functional mobility and safety
Activities of daily living (ADLs)
Recommend, fabricate, and maintain adaptive equipment and hand splints as needed
Assess occupational performance related to self-care, work, leisure, and rest
Evaluate sensory-motor, neuromuscular, psychosocial, and cognitive-perceptual functioning
Assess physical, social, and cultural components of the patient’s environment
Apply infection control standards and universal precautions at all times
Communicate effectively with physicians, caregivers, and the care team regarding patient status and progress
Maintain accurate and timely clinical records and documentation
Support compliance with third-party payer and billing requirements
Perform basic troubleshooting, cleaning, and setup of occupational therapy equipment
Complete other related duties as assigned
Required Qualifications
Current state license as an Occupational Therapist
Graduate of an accredited occupational therapy program (Bachelor’s degree or higher in Occupational Therapy)
Current CPR certification
Valid driver’s license
Reliable transportation and current insurance
Minimum of two (2) years of experience as an Occupational Therapist in an acute care or rehabilitation setting

Ability to work independently in a home care environment
Ability to work a flexible schedule and travel locally
Demonstrated commitment to professional growth and continuing education
Ability to perform physical demands of the role, including lifting, bending, pushing, pulling, prolonged standing, and walking
Ability to work under time-sensitive and sometimes stressful conditions
Strong hand-eye coordination and manual dexterity
