This opportunity is ideal for an experienced pharmacist seeking to provide comprehensive pharmaceutical care in a 50-bed hospital setting in Northern Wisconsin. The role involves working on a dynamic 7-on, 7-off schedule with 12-hour day shifts, ensuring work-life balance while delivering expert pharmacy services.
Key Responsibilities:
- Review medication orders to assess drug therapy appropriateness, dosage, regimen, and potential drug interactions.
- Provide pharmacokinetic consultations and anticoagulation dosing support.
- Engage actively in clinical services, including medication reconciliation and discharge medication education.
- Oversee pharmaceutical compounding, ensuring compliance with USP regulatory standards, focusing on USP 797 and USP 800 guidelines.
- Utilize EPIC electronic health records to manage medication orders and documentation efficiently.
Qualifications and Desired Experience:
- Current pharmacist license with relevant state credentials.
- Proven experience in hospital pharmacy settings.
- Strong knowledge of pharmacokinetics, anticoagulation therapy, and pharmaceutical compounding regulations.
- Familiarity with USP 797 and USP 800 standards.
- Proficiency with EPIC electronic health systems preferred.
