The Adult Day Care LPN will provide essential nursing care within a supportive, small-team environment, focusing on elderly adults, including those with dementia and physical disabilities. This role requires moderate independence in clinical decision-making and involves working with an advanced electronic health record system. The LPN will be a key contributor to patient wellness and safety while engaging in ongoing professional development.

Responsibilities

• Deliver patient care tailored to elderly and adult day program participants
• Administer medications safely and accurately
• Monitor health status, including vital signs and general condition
• Maintain thorough and accurate care documentation in EHR systems
• Communicate effectively with patients, families, and interdisciplinary team members
• Collaborate within a small team of 2-4 members to ensure continuity of care
• Adhere to all safety and regulatory compliance standards

Preferred Qualifications

• 2+ years of experience in adult care nursing
• LPN Certification and Licensed Practical Nurse (LPN) License
• CPR and Basic Life Support (BLS) certification
• Proficiency in medication administration and vital signs monitoring
• Strong care documentation and communication skills
• Demonstrated teamwork and time management abilities
