About the Role: The Respite Clinical Supervisor plays a pivotal role in overseeing the delivery of high-quality clinical care within the Medical Respite Program. This position is responsible for ensuring that patients experiencing homelessness or unstable housing receive comprehensive, compassionate, and coordinated medical respite services that promote recovery and stability. The supervisor will lead and support a multidisciplinary clinical team, fostering a collaborative environment that prioritizes patient-centered care and adherence to best practices. They will also be instrumental in developing and implementing clinical protocols, monitoring program outcomes, and ensuring compliance with regulatory standards. Ultimately, the role aims to enhance patient health outcomes and facilitate successful transitions to long-term housing and healthcare solutions.

Minimum Qualifications:

  • Minimum of 3 years of clinical experience in nursing or a related healthcare field, preferably in community health or social assistance settings.
  • At least 2 years of supervisory or leadership experience in a clinical environment.
  • Strong knowledge of medical respite care principles, patient care standards, and healthcare regulations.
  • Excellent communication, organizational, and interpersonal skills.

Preferred Qualifications:

  • Experience working with vulnerable populations, including individuals experiencing homelessness.
  • Familiarity with case management, care coordination, and trauma-informed care approaches.
  • Proficiency with electronic health records (EHR) systems and data reporting tools.

Responsibilities:

  • Supervise and provide clinical guidance to healthcare professionals within the Medical Respite Program.
  • Coordinate interdisciplinary team meetings to review patient progress, address challenges, and plan individualized care strategies.
  • Ensure compliance with healthcare regulations, accreditation standards, and organizational policies.
  • Provide direct clinical support and consultation for complex patient cases as needed.
  • Facilitate staff training, professional development, and performance evaluations to maintain a skilled and motivated clinical team.
  • Collaborate with community partners, healthcare providers, and social service agencies to support patient transitions and resource linkage.
