Job DescriptionJob Title: Travel LPN / LVN (Long Term Care – LTC)Location: Rochester, NY 14612Assignment Length: 8 WeeksSchedule: 42.5 Hours/WeekShift: Evening Shift | 3:00 PM – 11:30 PM (5x8-Hour Shifts)Weekends: Every Other Weekend (Total 8 weekend shifts during assignment)Employment Type: Travel ContractWork Setting: OnsiteUnit: Nursing Administration (LPN)Position Summary:

We are seeking a dependable and compassionate Travel LPN/LVN to provide high-quality nursing care in a Long Term Care (LTC) and Rehabilitation setting. The ideal candidate will have prior LTC experience and be comfortable working in a fast-paced environment while managing a diverse patient population.

Key Responsibilities:

  • Provide direct nursing care to long-term care and rehabilitation patients
  • Administer medications and treatments as prescribed
  • Monitor and document vital signs and patient health status
  • Perform blood glucose monitoring and related care
  • Assist patients with activities of daily living while ensuring comfort and safety
  • Accurately document patient information in EMR systems (Epic preferred)
  • Communicate effectively with physicians, RNs, and interdisciplinary teams
  • Float across all units within the facility as needed
  • Follow infection control and safety protocols
  • Respond promptly to changes in patient condition
  • Maintain a professional and compassionate approach to patient care

Required Qualifications:

  • Active LPN/LVN license (New York or compact state if applicable)
  • Minimum 1 year of LPN experience in Long Term Care (LTC)
  • Current BLS certification
  • Experience in Skilled Nursing Facility or Nursing Home settings
  • Familiarity with EMR systems (Epic preferred)

Skills & Competencies:

  • Strong clinical and patient care skills in LTC environments
  • Experience with blood glucose testing and monitoring
  • Ability to manage high patient ratios (up to 1:40 on nights)
  • Excellent communication and teamwork skills
  • Strong documentation and organizational abilities
  • Flexibility to float between units

Additional Requirements:

  • First-time travelers are welcome
  • Must reside more than 75 miles from the facility (travel requirement)
  • Ability to commit to the full 8-week assignment
  • Scrub Color: Any
