Shift: Day Shift – 5x8 Hours (07:00–16:00 or 08:00–16:00)

Job Description:

We are seeking a dedicated and experienced Registered Nurse Case Manager to join a dynamic acute care hospital setting. The ideal candidate will coordinate patient care, ensure efficient utilization of resources, and support optimal patient outcomes through effective discharge planning and case management.

Key Details:

• Experience Required: Minimum 1 year
• First-Time Travelers: Accepted
• Schedule: 5x8 Day shifts (timing may vary)
• Weekend Requirement: Every other weekend or 4 weekend days in a 4-week schedule
• On-Call Requirement: None
• Floating: May be required within scope based on facility needs
• Guaranteed Hours: 72 hours per 2 weeks (facility may cancel up to one 8-hour shift per 2 weeks)

Required Qualifications:

• Active RN License (state-specific requirement)
• BLS Certification
• Acute Care experience
• Experience with Epic charting system

Preferred Qualifications:

• ACM (Accredited Case Manager) Certification
