- Minimum 2 years of recent experience as a Home Health Registered Nurse
- Proficiency with OASIS and HCHB (Homecare Homebase) documentation systems
- Current CPR certification
- Valid driver’s license, vehicle insurance, and reliable transportation

- Deliver skilled nursing care to patients in their homes, following individualized care plans
- Perform comprehensive patient assessments and document findings using OASIS and HCHB
- Collaborate with interdisciplinary healthcare team members to ensure optimal patient outcomes
- Educate patients and families on disease management, medications, and self-care techniques
- Monitor patient progress, update care plans, and communicate changes to the healthcare team
- Maintain accurate, timely clinical documentation in compliance with home health regulations
