Compensation
-
Travel Rate: $48 – $51 per hour

Gross Weekly: $1,920 – $2,040 (based on experience)

Local Rate: $40 – $45 per hour

Gross Weekly: $1,600 – $1,800 (based on experience)

Position Summary
A healthcare facility in Falmouth, MA is seeking a Licensed Practical Nurse (LPN/LVN) – Long-Term Care for a contract assignment. The LPN will provide direct nursing care to residents and supervise nursing assistants while ensuring compliance with federal, state, and local long-term care regulations.
The ideal candidate will maintain high standards of patient care while supporting daily clinical operations in a long-term care environment.
Responsibilities

Provide direct nursing care to residents in a long-term care setting

Supervise and direct the daily activities of nursing assistants

Plan and coordinate shift services, resident care programs, and nursing activities
-
Complete required documentation for resident admissions, transfers, and discharges
-
Receive and document physician orders and treatment instructions
-
Transcribe physician orders into resident charts, medication records, and care plans
-
Maintain detailed nursing notes and clinical documentation reflecting patient care and outcomes

Collaborate with physicians and interdisciplinary teams to support resident treatment plans
-
Ensure compliance with facility policies, regulatory standards, and quality care guidelines
Required Qualifications
-
Graduate of an approved Licensed Practical Nurse (LPN) program
-
Minimum 1 year of Long-Term Care (LTC) experience
-
Active Massachusetts LPN License in good standing
-
BLS / CPR / AED Certification through the American Heart Association
-
Strong clinical, communication, and documentation skills
-
Ability to read, write, speak, and understand English effectively
Additional Requirements
-
Demonstrated knowledge and skills necessary to provide age-appropriate care for residents

Ability to work mixed shifts, weekends, and holidays as required
