PARTICIPANT CARE ASSOC. - DAY HEALTH CENTER LEAD

The Day Health Center Team Leader/Nursing Assistant (DHA) provides supportive services to the participant in the day health center and community setting, under the supervision of the Day Health Center Manager. The Team Leader/Nursing Assistants responsibilities for each participant are specified in the Center Nursing Assistant Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. The list should not be taken as an exhaustive list of all responsibilities, duties, and skills required of an individual assigned to this job.

SPECIFIC DUTIES AND FUNCTIONS:

  • The Day Health Center Team Leader/Nursing Assistant plan of care is based on the initial and on-going assessments that are performed by the RNCM and IDT.
  • The Day Health Center Team leader/Nursing Assistant assist in providing and/or maintaining optimal physical and emotional comforts to frail elders at the day health center, in the participant’s home, and community setting.
  • The Day Health Center team Leader/Nursing Assistant assist participants toward achieving maximum self-reliance and independence within the Day Health Center and community setting.
  • The Day Health Center Team Leader/ Nursing Assistant performs the care or services that are outlined in the Center Nursing Assistant care plan; any care Guidelines that are pertinent diagnosis are specified in the care/service plans.
  • The Day Health Center Team Leader/ Nursing Assistant performs a variety of tasks and services for the participant , including:
    • Personal care
    • Assistance with activities
    • Escort during medical appointments; assisting as needed
    • Assistance with meal preparation/feeding
    • Assistance with toileting /managing incontinence
    • Simple treatments
    • Medication reminders
    • Laundry service
    • Sweep floors and clean tables in Day Health Center
    • Ensure that wheel chairs are returned into storage at end of day
    • Provide supervision under the direction of the DHC manager for the center Aides
    • Provide in-services
    • Competencies and precept new hires
    • Maintain daily flow of the DHC
    • Provide coverage for daily apt
    • Assist with the daily flow of transportation drop off and end of the day load
    • Monitor documentation of PACE
  • The Team Leader/Day Health Center Nursing Assistant is responsible for accurate documentation of the care and/or series provided each day/visit. On the Center Nursing Assistant progress not. Day health care/service must also be documented in the Data PACE binder. The Day health Nursing Assistant keeps the nurse informed of the participants progress.
  • The Team Leader/Day Health Center Nursing Assistant responds to emergency situations in accordance with established policies and procedures.

KNOWLDEGE, SKILLS AND ABILITIES:

  • Must be legally authorized to practice in Michigan. Certification required. One year nursing assistant experience required. Experience must be in a long term care, hospital, or home care setting. Must have one (1) year of experience with a frail or elderly population. Only acts within the scope of his/her authority to practice
  • The Team Leader/Day Health Center Nursing Assistant participates In annual, mandatory in- service training and screening, including but not limited to infection control, TB testing, safety training and BLS training.
  • The Team Leader/Day Health Center Nursing Assistant must possess the ability to establish and maintain interpersonal and interdepartmental relationships.
  • The Team Leader/Day Health Center Nursing Assistant shows respect for the physical, spiritual, and well-being of participants and co-workers.
  • The Team Leader/Day Health Center Nursing Assistant must have the ability to problem solve and interpret instructions.
  • The Team Leader/Day Health Center Nursing Assistant must be able to organize and maintain flexible scheduling to coordinate job responsibilities.
  • Must meet a standardized set of competencies (approved by CMS) after working independently.
  • Must be medically cleared for communicable diseases and have all immunizations up-to-date after engaging in direct participant contact.
  • The Team Leader/Day Health Center Nursing Assistant must be able to perform duties in atmosphere of frequent interruption.
  • The Team Leader/Day Health Center Nursing Assistant competency is monitored and maintained in a variety of ways:
    • Initially, through skills check –off performance evaluation of specific tasks upon hiring and orienting to the program.
    • Job related competency in-services including a practicum demonstration and written assessment
    • Mandatory in-service training
    • Department staff meetings

WORKING CONDITIONS:

  • Working conditions are variable. Possible exposure to communicable diseases, due to participant environment. There is also potential for exposure to chemicals such as cleaning solutions and medications.
  • The Team Leader/Day Health Center Nursing Assistant provides care/services under the direct daily supervision of the DHS manager.
  • The Team Leader/Day Health Center Nursing Assistant must have reliable transportation available on a daily basis, and must maintain an acceptable driving record according to CSI policy.
  • The Team Leader/Day Health Center Nursing Assistant must have the ability to transfer and assist with moving patients; frequent walking. Bending and lifting of forty (40) pounds or more may be needed in the performance of duties.
  • The Team Leader/Day Health Center Nursing Assistant may be required to change participant assignments based on changing participant acuity, participant needs, and participant census.
