We are seeking a dedicated and compassionateHospice Intake Coordinator whose heart for service is matched by a commitment to patient care and client satisfaction. This is more than a clerical or administrative role—it is a calling to be the first point of contact for patients, families, and referral sources, ensuring that every inquiry is handled with efficiency, professionalism, and compassion. As our Hospice Intake Coordinator, you will manage all incoming hospice referrals and work directly with our hospice sales team to facilitate timely and seamless care for patients.

As Our Hospice Intake Coordinator, Here’s a Big Picture View of What You’ll Do:

  • Receive, log, and coordinate all incoming hospice referrals.
  • Serve as the primary point of contact for patients, their families, referral sources, and external agencies.
  • Provide clear, accurate information to patients/clients and referral sources to assist in accessing appropriate hospice services.
  • Conduct complete intake screenings and ensure accurate data entry into the management information system.
  • Collaborate with the hospice sales team and clinical staff to schedule patient visits and Hospice RN admissions.
  • Prepare reports, respond to inquiries, and complete audits as required.
  • Actively promote hospice services to prospective patients and referral partners.
  • Where permitted by law, receive verbal orders from physicians to initiate care/services.
  • Maintain access to personal health information (“PHI”) in accordance with HIPAA and company policies.
  • Actively participate as part of a high-performance team to drive change and deliver exceptional patient/client service.

A few must-haves for our Hospice Intake Coordinator:

  • Current Licensed Vocational Nurse (LVN) in the state of California.
  • One (1) year of experience in intake coordination, hospice, or home health preferred.
  • Computer proficiency, including the ability to learn company software.
  • Strong knowledge of hospice philosophy and patient-centered care practices.
  • Excellent communication, customer service, and organizational skills.
  • Ability to work independently and collaboratively within a team.
  • Demonstrated professionalism, attention to detail, and ability to prioritize multiple tasks in a fast-paced environment.
