Job Description
A healthcare organization in Suffolk County, NY is seeking a Licensed Practical Nurse (LPN) to join its Residential Program team. This role involves providing healthcare support for adults with developmental disabilities in a community setting.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 7am to 3pm or 8am to 4pm or 9am to 5pm
- Setting: Community residences
Responsibilities:
- Attending and accompanying residents to medical appointments
- Preparing necessary paperwork and ensuring follow-up documentation
- Scheduling and confirming medical consults
- Maintaining up-to-date health files for residents
- Communicating appointment results to families and the interdisciplinary team
- Ensuring availability and tracking of medications and health supplies
- Reviewing Monthly Medication Administration Records (MARs)
- Coordinating with pharmacies for medication access
- Supporting compliance with OPWDD standards
- Collaborating with the supervising RN and site staff
- Participating in team meetings and monthly updates
- Shadowing current staff and covering sites independently once trained
Qualifications:
- Active NYS Licensed Practical Nurse (LPN) license required
- Strong attention to detail and documentation skills
- Excellent communication skills
- Compassionate, patient-centered approach to care
- Ability to work independently and punctually
Desired Skills:
- Experience in ID/DD settings or knowledge of OPWDD regulations preferred
