Position Summary

At Concourse Rehabilitation and Nursing Center, we are committed to delivering exceptional, patient-centered care in a dynamic and supportive environment. The Director of Rehabilitation (DOR) serves as the clinical and operational leader of our therapy department, overseeing physical therapy, occupational therapy, and speech-language pathology services.

This leadership role is open to qualified clinicians from Physical Therapy, Occupational Therapy, or Speech-Language Pathology, bringing diverse clinical perspectives to drive excellence in care.

The DOR is responsible for advancing clinical quality, developing innovative rehabilitation programs, ensuring regulatory compliance, and fostering a collaborative, high-performing team culture dedicated to outstanding patient outcomes.

Key Responsibilities

Leadership & Program Development

  • Lead, mentor, and supervise all rehabilitation staff across PT, OT, and Speech Therapy.

  • Develop and implement high-quality, outcomes-driven rehabilitation programs in collaboration with facility leadership and medical staff.

  • Champion innovative programming, including restorative nursing, specialty rehab pathways (e.g., orthopedic, cardiac), and patient-centered initiatives.

  • Integrate rehabilitation services seamlessly into the overall resident care experience.

Operations & Compliance

  • Partner with Regional and corporate leadership to manage staffing, budgeting, and departmental performance.

  • Ensure full compliance with federal, state, and professional practice standards.

  • Maintain and update departmental policies and procedures in alignment with regulatory requirements.

  • Proactively resolve operational challenges and escalate issues when appropriate.

Clinical Oversight & Quality Assurance

  • Conduct monthly Quality Assurance (QA) reviews and chart audits for all rehabilitation patients.

  • Ensure accuracy, timeliness, and compliance of all clinical documentation.

  • Address deficiencies through staff education, coaching, and progressive discipline as needed.

  • Lead Quarterly Program Evaluations to assess performance, identify gaps, and implement action plans.

Technology & Performance Management

  • Utilize Net Health to:

    • Monitor therapy delivery and clinical outcomes

    • Maintain daily HOTLIST oversight

    • Complete monthly closeouts and billing exports

    • Track Medicare Part B thresholds and ensure compliance

  • Run and analyze management reports, including:

    • Therapist productivity

    • Average length of stay

    • Functional outcome measures

    • Census summaries

  • Leverage Business Intelligence dashboards to monitor:

    • Compliance

    • Operations

    • PDPM performance

    • Productivity and efficiency

    • DOR toolkit metrics

Staff Development & Culture

  • Oversee onboarding, orientation, and ongoing training of rehabilitation staff.

  • Complete and maintain competency evaluations for all team members.

  • Identify staff education needs and coordinate in-services and professional development opportunities.

  • Foster a positive, team-oriented culture focused on excellence, accountability, and continuous improvement.

Admissions, Discharges & Program Evaluation

  • Review referrals and admissions to ensure appropriate service utilization.

  • Evaluate discharges and outcomes to continuously improve care delivery.

  • Assess rehabilitation and long-term care program effectiveness and recommend enhancements.

Communication & Leadership Presence

  • Provide regular updates to the Administrator on departmental performance, challenges, and opportunities.

  • Present and interpret monthly Quality Assurance and operational reports.

  • Act as a key member of the interdisciplinary leadership team.

Marketing & Growth

  • Support facility census and growth initiatives through:

    • Rehabilitation screenings

    • Community outreach and referral development

    • Contribution to marketing materials highlighting outcomes and program success

  • Participate in market analysis and recommend new rehabilitation programs aligned with community needs.

Patient Care Responsibilities

  • Provide direct patient care based on facility needs and productivity expectations.

  • Maintain discipline-specific clinical standards consistent with licensure and scope of practice.

Schedule Expectations

  • Due to the nature of healthcare services, this role may include weekend coverage as part of the regular schedule.

Qualifications

  • Bachelor’s or Master’s Degree in Physical Therapy, Occupational Therapy, or Speech-Language Pathology

  • Current or eligible for New York State licensure in Physical Therapy, Occupational Therapy, or Speech-Language Pathology

  • Minimum of 3+ years of rehabilitation experience

  • Proven leadership and supervisory experience in a healthcare setting

  • Strong understanding of Skilled Nursing Facility (SNF) operations, PDPM, and regulatory compliance

  • Knowledge of state, federal, and therapy practice regulations

  • Excellent communication, organizational, and problem-solving skills

  • Ability to lead interdisciplinary teams and drive program development

  • Skilled Nursing Facility experience strongly preferred
