Job Description
The Certified Nursing Assistant (CNA) at The Terrace at Crystal provides direct resident care and assists residents with activities of daily living (ADLs) under the supervision of a licensed nurse. The CNA supports resident rights, dignity, safety, and quality of life while complying with all federal, state, and facility regulations, including CMS requirements and state survey standards.
Essential Duties and ResponsibilitiesResident Care

Assist residents with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, eating, mobility, and transfers.

Promote resident dignity, privacy, independence, and choice at all times.

Observe, monitor, and promptly report changes in resident condition, behavior, or safety concerns to the licensed nurse.

Assist with positioning, turning, and skin care to prevent pressure injuries.

Provide emotional support and reassurance to residents and families.
Safety and Infection Control
-
Follow all infection prevention and control protocols, including hand hygiene, PPE use, and isolation precautions.

Maintain a safe environment by identifying and reporting hazards, unsafe conditions, or malfunctioning equipment.

Follow fall-prevention, abuse prevention, and elopement-prevention protocols.
Documentation and Communication

Accurately document care provided, observations, and resident interactions in the electronic health record (EHR) or approved documentation tools.

Participate in shift reports, communication logs, and care conferences as assigned.

Maintain confidentiality and resident privacy in accordance with HIPAA and facility policy.
Regulatory Compliance and Quality

Adhere to all CMS regulations, Minnesota Department of Health requirements, and The Terrace at Crystal policies and procedures.

Participate in required education, annual training, in-services, and competency evaluations.

Support Quality Assurance and Performance Improvement (QAPI) initiatives.

Demonstrate professionalism and regulatory awareness during state surveys and audits.
Teamwork and Professional Conduct

Work collaboratively with licensed nursing staff and the interdisciplinary care team.

Follow the chain of command and report concerns appropriately.

Maintain respectful, professional, and ethical conduct at all times.
Non-Essential Duties
-
Assist with resident transportation to activities, therapy, or appointments as assigned.

Perform other duties within the CNA scope of practice as assigned.
QualificationsRequired
-
Current Certified Nursing Assistant (CNA) certification in the State of Minnesota.

Active status on the Minnesota Nurse Aide Registry.

Ability to read, write, and communicate effectively in English.
-
Ability to follow written and verbal instructions.
-
Successful completion of required background checks, health screenings, and TB testing per state and facility requirements.
Preferred
-
Prior experience in a Skilled Nursing Facility (SNF) or long-term care setting.
-
Knowledge of resident rights and person-centered care principles.
-
Experience with electronic medical records (EMR/EHR) systems.
Physical Requirements

Ability to lift, push, pull, and transfer residents with or without assistance.

Ability to stand, walk, bend, stoop, kneel, and reach for extended periods.
-
Ability to respond appropriately to emergencies.
