📋 Responsibilities
- Provide direct physical therapy interventions under PT supervision
- Follow and implement the PT’s care plan
- Document treatment, patient progress, and response
- Support and instruct patients, families, and caregivers
- Monitor patient outcomes and assist with reassessment
- Ensure compliance with all state/federal regulations and facility policies

- Resume with full education (MM/YYYY) & 7-year work history (must list facility names, not agencies)
- 2 recent references (1 must be a supervisor from the last 3 years)
- Skills checklist (completed within 1 year)
- License verification (Nursys or equivalent within 30 days of submission)
- Background check (7-year criminal check, FACIS III, sex offender, all states lived/worked, aliases)
- Virginia State Police Background Check
- Drug screen: 9-panel, HHS-certified lab, within 1 year (no marijuana testing required)
- Physical exam: within past 12 months (signed by MD, DO, PA, or NP)
- TB screening: Annual PPD, QuantiFERON, or T-Spot; chest x-ray every 5 years if positive
- MMR, Varicella, Hep B: 2 doses or positive titer; declination allowed after equivocal results
- Flu Vaccine: Required Oct 15 – Mar 31 (declinations allowed)
- Core Competency Exams I–III: 80% passing score, valid for 1 year
