- General job duties include patient education, head to toe assessments, vital signs monitoring, skin/wound assessments, dressing changes, wound care, PICC line care, lab draws, foley catheter changes, ostomy care, medication reconciliation, and Epic documentation.
- Interdisciplinary care coordination with providers, therapy staff, social work, wound centers, and infusions as appropriate.
- Equipment utilized includes laptops, portable printers, and cell phone communication.
- Job Requirements
- State of Illinois RN license
- BLS certification
- At least 1 year experience as a Home Health RN
- Patient assessment skills
- Medication management
- Wound care
- IV competency – PICC, peripheral, etc.
- Lab draws
- Point of care testing – INR
- Foley catheter and suprapubic catheter care
- Ostomy care
- Communication skills
- Patient and family education
- Schedule Information
- Clinician sees an average of 5 patients per day.
- Hours are 8:00 am – 4:30 pm, Monday – Friday.
- On call weeknights and weekends on a rotation basis.
- Work every 4th weekend, regular day shift.
- Unit Specific Information
- Staff development includes 1:1 training, wound care, IV class, training on Canvas platform, Health Stream (online modules), and general orientation. Approx. 5 – 7 days.
