About the Role


We are seeking a compassionate and experienced Director of Nursing (DON) to lead our hospice care team. This role is central to ensuring that patients and families receive the highest quality of end-of-life care, while maintaining compliance with state, federal, and accreditation standards.


Key Responsibilities

  • Provide leadership and oversight for all hospice nursing staff.
  • Ensure delivery of patient-centered, compassionate care in alignment with hospice philosophy.
  • Develop, implement, and evaluate nursing policies, procedures, and standards of care.
  • Supervise, mentor, and evaluate nursing staff performance.
  • Collaborate with interdisciplinary team members (physicians, social workers, chaplains, volunteers).
  • Manage staffing schedules, caseload assignments, and on-call rotations.
  • Review, implement, and oversee physician orders for medications and treatments.
  • Ensure accurate administration, documentation, and monitoring of prescribed medications.
  • Coordinate medication management, including reconciliation, patient/family education, and compliance with regulatory standards.
  • Monitor compliance with regulatory requirements (Medicare, state licensure, accreditation).
  • Participate in quality improvement initiatives and staff education programs.
  • Serve as a liaison with patients, families, and community partners.

Requirements:

QUALIFICATIONS

  • Current Registered Nurse (RN) license in Minnesota.
  • Bachelor’s degree in nursing (BSN required; MSN preferred).
  • Minimum 3–5 years of hospice or palliative care experience, with at least 2 years in a supervisory role.
  • Strong knowledge of hospice regulations and standards of practice.
  • Excellent leadership, communication, and organizational skills.
  • Valid driver’s license and reliable transportation.
