Qualifications

  • Graduate of a vocational/technical nursing program
  • Current LVN/LPN license in the state of Texas
  • Knowledge of lab procedures, diagnostic testing, and EMR systems
  • Strong documentation, communication, and organizational skills

Key Responsibilities

  • Review medical records, identify gaps in care, and schedule preventive services per physician-approved protocols
  • Collaborate with providers, Nurse Navigators, and families to ensure continuity of care and patient support
  • Educate patients using evidence-based guidelines to promote engagement and health compliance
  • Maintain documentation accuracy, confidentiality, and compliance with all clinical and regulatory standards
