Pharmacy Medication Reconciliation Technician

Job Summary:

The Pharmacy Medication Reconciliation Technician (PMRT) is responsible for accurately collecting and entering current patient medications into the hospital’s electronic health record (EHR) for utilization by the physician responsible for admitting and discharging the patient to and from the hospital. The PMRT is responsible for relaying any issues that cannot be addressed to the pharmacist and/or physician. The PMRT will also process discharge prescriptions from the ED and other areas using the Retail Pharmacy Computer System. The PMRT may perform limited medication management and distribution functions under the direction of the pharmacist.

Reports to: Director of Pharmacy, Pharmacy Clinical Manager

Education: High School diploma or GED

License: State of Ohio Board of Pharmacy Technician Certification

Experience: 2-3 years of pharmacy technician experience; retail pharmacy technician experience

preferred, but not required

Work Schedule:

  • Monday through Friday 1:00 P.M.to 9:30 P.M. Weekend hours to be determined. Hours and days are subject to change as the program expands

Physical Requirements:

Repetitive use of hands and fingers (e.g., use of a computer keyboard); may require lifting and carrying light loads (up to 40 pounds), including boxes, equipment, unit-dose cassettes, and IV solutions and stooping or kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves, and to file documents in lower file drawers); sitting, walking, or standing for long periods of time (4-8 hours) is often necessary; must be able to physically operate the equipment used for the job

  • Submission to a BCI and FBI criminal background check which shows no previous conviction or has not pleaded guilty to any felony in the U.S.
  • No written disciplinary action within the last 12 months

  • Familiarity with allergic reactions and medication intolerances
  • Attention to detail
  • Self-motivated; able to work unsupervised

Occupational Hazards:

Potential for exposure to infectious patients and materials; exposure to violent patients; exposure to hazardous and toxic substances including chemotherapy, cytotoxic drugs, and cleaning solutions; potential for sticks or cuts by needles and other sharp items; potential for musculoskeletal injuries if proper lifting and carrying techniques are not used.

Principal Duties / Responsibilities:

  1. Obtain a patient medication history:
    1. Interview the patient and any family members present to obtain an accurate list of medications the patient takes every day for those patients that will be admitted to the hospital
    2. The list should include prescription, non-prescription (over-the-counter) herbal medications as well as vitamins and other nutritional supplements
    3. Document the medication list accurately in the EHR, using correct doses, dosage forms, directions and schedules
    4. Resolve discrepancies in the med list by contacting the patient’s physicians and pharmacies in a timely fashion (preferably before the patient leaves the Emergency Department)

  1. Process Discharge Prescriptions for ED Patients
    1. Process discharge prescriptions using the Retail Pharmacy Computer System
    2. Maintains fluency in prescription insurance information processing
    3. Operate the remote check out register

  1. Obtain patient’s allergies:
    1. Interview the patient and any family present to obtain an accurate list of the patient’s allergies and intolerances for those patients that will be admitted to the hospital
    2. The list should include medication, food, environmental allergies and intolerances and their reactions
    3. The list should be entered in the EHR in a correctly codified format to allow for correct screening of orders
    4. Every effort should be made to include the reaction to the allergy or intolerance as well as entering comments in the HER

  1. Perform the monthly ED Unit Inspection:
    1. Responsible for the expiration dates of all medications in the Emergency Department within and outside of automated distribution cabinets
    2. Responsible to ensure all medications are secure in the ED
    3. Completes the monthly unit inspection form and submits to the Director of Pharmacy
  1. Facilitates restocking of ED kits / crash cart drug drawers:
    1. Used kits / boxes will be returned to Pharmacy for restocking
    2. Responsible to return kit to proper storage location in the ED
    3. Responsible to exchange the drug drawers in code cart after a code.
    4. Responsible to ensure a patient’s name is provided to the Pharmacy when exchanging the drug drawers

  1. Restocks ED automated distribution cabinets
    1. Obtains medications needed to be restocked from Pharmacy
    2. Restocks the ED and EDMM ADCs
    3. Retrieves stock outs for ED ADCs from Pharmacy as time permits
    4. Ensures controlled substance medication discrepancies are resolved in a timely fashion (before end of shift)

  1. Expedite delivery of critical medications to ED
    1. As time permits, serves as courier during Stroke Team, STEMI Alerts, and other situations where time critical medications are needed
