Job DescriptionPhysical Therapist (PT) – Facility-Based | Port Jervis, NY | PRN & Part-Time | $50–$65 Per Visit
Stern at Home Therapy is hiring a Physical Therapist (PT) to provide one-on-one physical therapy services at a single facility in Port Jervis, NY.
Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability.
This role offers the consistency of an established patient caseload without the driving and unpredictability of traditional home health. There are no home visits and no travel between patients — you work at one location, treat one patient at a time, and receive support from our administrative and clinical teams.
What You'll Do:As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat patients at a single facility in Port Jervis.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one treatments based on each patient's functional needs and goals.
- Manage individualized treatment plans with clinical autonomy.
- Help patients improve mobility, strength, balance, safety, and overall function.
- Complete accurate and timely clinical documentation.
- Communicate with patients, families, facility staff, physicians, and members of the clinical team.
- Coordinate with our administrative team regarding scheduling, patient care, and logistics.
- Manage your caseload based on your availability and patient needs.
Compensation:Part-Time W-2:
- $50-55 per follow-up visit.
- $60 per 4-unit evaluation.
PRN:
- $60 per 4-unit follow-up visit.
- $65 per evaluation.

Qualifications:
- Active Physical Therapist (PT) license in New York required.
- Reliable transportation required.
- Minimum of one year of U.S. clinical experience preferred.
- CPR certification preferred.
- Strong clinical, communication, and time-management skills.
- Ability to work independently while collaborating with facility staff and members of the clinical team.
