Licensed Practical Nurse (LPN):

• Competitive rate per visit
• Rates are based on experience, case load, coverage area and skills

Registered Nurse (RN):

• Competitive rate per visit
• Start of Care/Resumption of Care and Recertifications are paid at a higher rate

Job Description

Secure Home Health Care is continuously seeking and hiring per-diem nurses (LPN/RN) to conduct home visits on behalf of our Home Health Agency.

Job Responsibilities:

• Assess patients’ condition during every visit and chart your observations
• Perform evaluation tasks, including vital signs and medication review
• Administer medication as prescribed by the patient’s Physician
• Dress or redress open wounds and assess progress of healing
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with other care providers involved in the patient’s care plan
• Listen to the patient and respond to concerns or request

Basic Requirements:

• State RN/LPN license (Required)
• CPR Certificate
• 1+ years’ experience in Home Care/VNA or inpatient or critical/palliative care
• Ability to travel to multiple patients per day (Required)
• Excellent communication, documentation and time-management skills
• Compassionate and friendly demeanor
