The Referral Coordinator is responsible for coordinating all rehab admissions to our Skilled Nursing Facilities. Completes pre-admission screening, which may include on-site assessment, calculating cost of care and identifying barriers to safe discharge. Communicates with inter-disciplinary team members at the hospital and at our SNFs to ensure a smooth transition. Knowledgeable on Medicare/Medicare Advantage/commercial insurance criteria for admission and obtains insurance authorizations when needed. Serves as a resource to hospital partners to promote Bethesda and its services.
Qualifications
- Bachelor's degree in related field preferred
- Previous Health Care/Clinical experience
- Previous hospital and/or post-acute setting experience
- Marketing skills
Pay range: $52,500 - $78,749 annuallyCategory: Business Development
