Compass Health is seeking a compassionate Care Coordinator to support youth and families enrolled in our Wraparound with Intensive Services (WISe) program in Everett. This role helps maintain children in their homes and communities by providing flexible, individualized, and trauma-informed care coordination.
About the WISe Program
WISe (Wraparound with Intensive Services) is a community-based approach supporting youth up to 21 years old with complex behavioral health needs. We meet youth and families in their homes, schools, and communities, helping them avoid out-of-home placements and build brighter futures.

What You’ll Do

Partner with families and natural supports to develop and implement individualized care plans

Provide case management, service coordination, advocacy, and linkage to resources

Deliver support in a variety of settings (home, office, school, community)

Work flexible hours including evenings, based on client needs

Collaborate with a multi-disciplinary team (including therapists, family/youth support partners)

Participate in a rotating after-hours crisis response schedule (if applicable)
What You Bring
-
Required: BA/BS/BSW in Behavioral Sciences or related field

Experience in behavioral health, medical, or social services field preferred

Knowledge of Evidence-Based Practices (or willingness to learn)

Ability to work effectively with families from diverse cultural backgrounds

Must have: WA Driver’s License, reliable vehicle, and current auto insurance

Must apply for Agency Affiliated Counselor status if not currently licensed/registered with WA DOH
-
Ability to pass a pre-employment background check
