Occupational Therapist (OT) - FT, PT, PRN

Responsibilities

• Perform patient evaluations and develop individualized treatment plans based on physician orders

• Deliver occupational therapy treatments and adjust plans as needed to achieve functional goals

• Supervise COTAs, rehab techs, and therapy students as required

• Maintain accurate and timely documentation in accordance with regulatory standards

• Collaborate with interdisciplinary team members, including nurses, physicians, and physical therapists

• Educate residents, families, and staff on therapeutic techniques and home exercise programs

• Participate in discharge planning and facility quality improvement initiatives

Qualifications

• Valid state licensure as a Physical Therapist

• Prior experience in long-term care is a plus

• Strong communication and organziational skills

• Passion for working with geriatric population
