ABOUT THE ROLE
This isn't a referral-handling job. You'll own contract cases end-to-end — from intake through discharge and 30-day follow-up — embedded directly inside UMMS with full EPIC access and a team behind you.
SilverStay partners with hospitals to solve complex discharges that internal teams can't close alone. In this role, you're our on-site presence at University of Maryland Medical System: attending bedside visits, coordinating ALF tours, managing 10–15 active cases at a time, and building trusted relationships with hospital discharge planning teams.
You're not starting from scratch — there's an active book of business waiting, with infrastructure and teammates already in place.
WHAT YOU'LL DO
- Own contract cases from referral intake through discharge — including options sourcing, family outreach, ALF coordination, and final placement
- Monitor EPIC and hospital EHR systems proactively to stay ahead of clinical changes before they become surprises
- Manage 10–15 active referrals simultaneously with thorough, timely documentation in SilverStay’s CRM
- Conduct in-person bedside visits and join ALF tours as needed; weekend availability is real and discussed upfront
- Build trusted working relationships with case managers, social workers, and discharge teams at UMMS
- Handle inbound hospital inquiries, logistical coordination, and referral follow-up so the broader team stays focused on sourcing and tours
- Provide follow-up with clients and families for a minimum of 30 days post-transition
WHO YOU ARE
Active MD Social Work license • 2+ yrs hospital discharge planning • EMR/EPIC experience (or fast learner)
Multi-stakeholder caseload management • 2+ placements/week track record • Baltimore-local with own transportation
Strong emotional intelligence • Self-directed; no micromanagement needed
