Job Title: Licensed Practical Nurse

Job Summary:
The Licensed Practical Nurse (LPN) functions as a member of the interdisciplinary health care team under the direction of the Supervisor, Nursing. This role provides high-quality patient care, supports clinic operations, and contributes to an exceptional patient experience through collaboration and adherence to best practices.
Duties/Responsibilities:
- Assists providers in delivering patient care under the supervision of RNs and/or providers.
- Explains procedures and treatments to patients to ensure understanding and cooperation.
- Aids providers during examinations and treatments.
- Works with the interdisciplinary health care team to facilitate care and maintain patient engagement through positive communication and teamwork.
- Administers treatments, nursing interventions, injections, and immunizations.
- Reinforces patient education on self-injection, medications, and other treatments.
- Rooms patients, takes vitals, and updates patient information.
- Collects samples (e.g., urine) and performs tests such as EKGs as required.
- Maintains exam rooms by cleaning between visits and stocking daily.
- Documents patient care clearly, accurately, and timely according to policy.
- Participates in quality improvement initiatives and quality assurance activities.
- Flexes as needed to cover LPN or MOA absences across different locations.
- Practices within the NYS LPN scope of practice and maintains annual competencies.
- Performs other related duties as assigned.
Required Skills/Abilities:
- Excellent verbal and written communication skills.
- Strong interpersonal and teamwork skills.
- Ability to prioritize tasks and manage time effectively.
- Proficient in Microsoft Office Suite; experience with EPIC EMR preferred.
- Demonstrates professionalism, stress management, and adaptability in a fast-paced environment.
Education and Experience:
- Current NYS LPN licensure and CPR certification required.
- Experience in community-based healthcare settings, pediatrics, and/or women’s health preferred.
- Experience with chronic disease management in an outpatient setting desired.
- Previous experience with Electronic Health Records; EPIC EMR preferred.
Physical Requirements:
- Work primarily performed in a clinic setting with occasional travel to off-site locations.
- Requires prolonged periods of sitting, computer work, and frequent verbal/written communication.
- Occasional standing, walking, bending, and reaching required.
- Ability to lift up to 50 lbs. as needed.
