JOB TITLE:
Client Care Coordinator
SUMMARY:
Responsible for the provision of supportive counseling with families residing in the Shelter
REPORTING RELATIONSHIP:
Position Reports to Director of Social Services
SCHEDULE:
Scheduled may vary based on program needs at the site
LOCATION:
Harlem, NY; Jamaica NY
POSITION TYPE:

ESSENTIAL DUTIES/RESPONSIBILITIES:
- Conduct Assessments: Perform comprehensive bio-psychosocial assessments and other clinical screenings to determine the need for further therapeutic interventions.
- Conduct psych-evaluations for the 2010E housing application, requiring strong knowledge of DSM-5 diagnostic criteria.
- Work with High-Need Families: Support families, especially those with members struggling with behavioral issues, mental health, substance abuse (SA), and intimate partner violence (IPV) using a Trauma Informed Care approach.
- Implement Evidence-Based Practices (EBPs): Use motivational interviewing techniques and harm reduction principles in therapy.
- Provide crisis intervention as needed and refer clients to the Housing Specialist for next-step housing needs.
- Work collaboratively with all staff within the facility and other providers to coordinate care.
- Ensure compliance with contract service requirements, maintain regular contact with families, and ensure their active participation in program activities.
- Provide clinical guidance and support to the social service team.
- Staff Training: Create and facilitate training sessions for staff.
QUALIFICATIONS:
LMSW, LMHC, MSW, MHC
OTHER SKILLS/ABILITIES:
- Attend divisional housing meetings, weekly staff meetings, quarterly agency meetings and other internal staff meetings as required.
- Work closely with clients to encourage compliance with programs Code of Conduct.
- Appropriate and timely referrals on behalf of clients, provides client advocacy in negotiating systems and delivery of services.
- Attend and or coordinate case conferences with other providers
- Other duties/tasks as assigned
