Description
American Traveler is seeking an experienced RN for a Med/Surg position at a skilled nursing facility in Lebanon, NH, with a focus on long-term and memory care residents requiring a compact RN license.
Details
- Skilled nursing facility (SNF) setting serving long-term care, sub-acute, and dementia/memory care units
- Patient population includes long-term residents requiring medication administration, memory care, and case management support
- Specialized care areas include PT, OT, substance use disorder (SUD), and pain management support
- EMR: PointClickCare (PCC)
- Night shift position with flexible shift lengths of 8, 12, or 16 hours available
- 40 hours per week
Requirements
- Active NH or compact RN license required
- Current BLS certification required
- Experience in skilled nursing, sub-acute, and dementia/memory care units required
- Proficiency in memory care techniques and protocols
- Knowledge of MDS (Minimum Data Set) assessments for long-term care facilities
- Strong background in case management practices
- Candidates must reside more than 75 miles from the facility to be eligible
- Two managerial references required for consideration
Additional Information
- Responsibilities include medication administration, resident health assessments, development and implementation of nursing care plans, and monitoring resident progress and changes in condition
- Familiarity with IV skills, IV medications, tube feedings, and light wound care is preferred but not required
- Strong interpersonal, communication, and task management skills are essential for this role
