We are Hampton Roads Primary Care, a primary care practice dedicated to providing high-quality, patient-centered healthcare to the community of Portsmouth, Virginia. We are seeking a compassionate and skilled Nurse Practitioner/Physician Assistant to join our team, part-time or full time, and contribute to our mission of improving the health and well-being of our patients.

Job Summary

As a Nurse Practitioner/Physician Assistant at our primary care practice, you will work collaboratively to provide comprehensive primary care services to our patient population. You will be responsible for diagnosing and treating a wide range of common health conditions, promoting preventive care, and educating patients on health management.

Key Responsibilities

  • Conduct patient assessments, including taking medical histories and performing physical examinations
  • Diagnose and treat common acute and chronic health conditions
  • Prescribe medications and other treatments within the scope of practice
  • Order and interpret diagnostic tests such as lab work and x-rays
  • Develop and implement treatment plans, emphasizing disease prevention and health promotion
  • Provide patient education on disease prevention, health maintenance, and lifestyle modifications
  • Collaborate with physicians and other healthcare professionals to ensure optimal patient care
  • Maintain accurate and detailed patient records in the electronic health record system
  • Participate in quality improvement initiatives and practice meetings
  • Stay current with evidence-based practices and continuing education requirements

Qualifications

  • Master's or Doctorate degree in Nursing (MSN) or Physician Assistant from an accredited program
  • Current Virginia state licensure as a Nurse Practitioner/Physician Assistant
  • National certification as a Family Nurse Practitioner (FNP-BC) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP-BC), or a Physician Assistant certified by the National Commission on Certification of Physician Assistants
  • DEA registration and prescriptive authority in Virginia
  • Minimum of 1 year of experience as a Nurse Practitioner in primary care (preferred)
  • Strong clinical skills and ability to manage complex patient cases
  • Excellent communication and interpersonal skills
  • Proficiency in electronic health record systems
