Job Summary: The LVN will provide direct patient care in a clinic setting under the supervision of a registered nurse or physician. This role includes administering medications, performing routine clinical procedures, and assisting with patient assessments.

Key Responsibilities:

  • Administer medications and treatments as prescribed by healthcare providers.

  • Conduct patient assessments, including taking vital signs and documenting patient information.

  • Assist with diagnostic tests and clinical procedures.

  • Provide patient education and support regarding treatment plans and health maintenance.

  • Maintain accurate and timely medical records.

  • Ensure a clean and safe clinical environment.

  • Assist in emergencies as needed.

  • Maintain clinic supplies and ensure adequate stock levels.

  • Actively participate in team meetings, contributing clinical expertise to team planning and problem-solving processes.

  • Must be willing to float throughout different facilities.

  • Perform other duties as assigned.

Required Qualifications:

  • Graduate of an accredited LVN program.

  • Current California LVN license in good standing.

  • Minimum one year of experience working with the elderly or frail population.

  • Ability to work independently and collaboratively within a multidisciplinary team environment.

  • Valid CA Drivers license

  • BLS Certification from the American Heart Association

  • Bilingual in Spanish, Cantonese, or any other language is highly preferred

  • Proficiency in electronic health records (EHR) systems.

Setting: Out Patient Clinic

Population: Geriatric population

Monday - Friday NO WEEKENDS
