Job Summary: The LVN will provide direct patient care in a clinic setting under the supervision of a registered nurse or physician. This role includes administering medications, performing routine clinical procedures, and assisting with patient assessments.
Key Responsibilities:

Administer medications and treatments as prescribed by healthcare providers.

Conduct patient assessments, including taking vital signs and documenting patient information.

Assist with diagnostic tests and clinical procedures.

Provide patient education and support regarding treatment plans and health maintenance.

Maintain accurate and timely medical records.

Ensure a clean and safe clinical environment.

Assist in emergencies as needed.

Maintain clinic supplies and ensure adequate stock levels.

Actively participate in team meetings, contributing clinical expertise to team planning and problem-solving processes.

Must be willing to float throughout different facilities.

Perform other duties as assigned.
Required Qualifications:

Graduate of an accredited LVN program.

Current California LVN license in good standing.
-
Minimum one year of experience working with the elderly or frail population.

Ability to work independently and collaboratively within a multidisciplinary team environment.

Valid CA Drivers license

BLS Certification from the American Heart Association

Bilingual in Spanish, Cantonese, or any other language is highly preferred
-
Proficiency in electronic health records (EHR) systems.
Setting: Out Patient Clinic
Population: Geriatric population
Monday - Friday NO WEEKENDS
