Job Description
We are hiring compassionate Registered Nurses (RNs) to join our AERS program team. In this role, you will provide in-home assessments and care coordination for adults and children with chronic or functional disabilities. Your work will help participants remain safely in their homes and avoid institutional care.

 Responsibilities: 
<ul>
<li>Conduct comprehensive in-home nursing assessments</li>
<li>Evaluate medical, functional, psychosocial, and environmental needs</li>
<li>Develop and update individualized care plans</li>
<li>Determine appropriate level of care and visit frequency</li>
<li>Coordinate services with care teams, families, and providers</li>
<li>Advocate for patient rights and independence</li>
<li>Document visits accurately in EMR systems</li>
<li>Monitor participant progress and outcomes</li>
<li>Travel to participant homes throughout Baltimore City</li>
</ul>
 
Requirements:
 
<ul>
<li>Active Maryland RN License (required)</li>
<li>Minimum 2 years of nursing experience</li>
<li>Med-surg, geriatrics, pediatrics, or case management</li>
<li>Valid driver’s license and reliable transportation</li>
<li>Ability to pass background screening</li>
<li>Comfortable with electronic medical records</li>
<li>Willing to travel locally for home visits</li>
