Job Title: Home Health Registered Nurse
Pay: $48.69 an hr | Bi-weekly
Job Type: Onsite in Gastonia, NC (Must be able to cover these counties (Gaston County, Lincoln County, Cleveland County, and zip code, 28214)
Must be travel for in person home visits
Shift Options:
- 8am-5pm (M-F, no weekend and no on-call) Holidays may be required weekday.
Home Health Registered Nurse Overview:
- The Registered Nurse (RN) is responsible for assessing the health of individuals served, implementing and supervising care plans, and promoting health maintenance and disease prevention. This role may also include case management responsibilities and supervision of care plans and staff members. The RN works collaboratively with interdisciplinary teams to ensure safe, effective, and person-centered care in community-based or small facility settings.
Key Responsibilities
- Observe, assess, plan, implement, and evaluate nursing care for well, chronically ill, or acutely ill individuals in community or small facility settings
- Implement and oversee individualized care plans in accordance with physician orders and organizational policies
- Provide case management and coordination of care as needed
- Work closely with physicians and other healthcare professionals to plan, implement, and evaluate programs and services addressing health needs
- Coordinate communication and actions among multiple disciplines related to health and safety supports
Medication Management
- Ensure medications are ordered, reviewed, monitored, discontinued, and disposed of as prescribed by the physician
- Ensure policies are in place for medication administration, medication errors, medication storage/access, and medication destruction
- Review and revise medication-related policies at least annually as needed
- Transcribe or ensure transcription of physician phone orders and other medically related reports
Documentation & Reporting
- Ensure progress reports are provided to physicians when a client’s condition changes
- Complete daily, monthly, quarterly, and annual nursing documentation as required
- Ensure routine documentation of individuals’ health status in nursing notes
Care Coordination & Follow-Up
- Schedule and ensure completion of health-related appointments
- Ensure timely and appropriate follow-up to appointment findings
- Maintain communication with inpatient nursing staff and discharge coordinators during hospitalizations
Infection Control & Safety
- Ensure furnishings, equipment, and supplies are maintained according to guidelines to prevent the spread of disease
Supervision & Training
- Direct and supervise Licensed Practical/Vocational Nurses (LPNs/LVNs) as needed
- Provide new employee training and annual recertification on health and safety-related topics
- Ensure unlicensed staff are trained on person-specific health maintenance, safety, and disease prevention needs
Additional Duties
- Perform other nursing-related duties as assigned to support quality care and organizational objectives
Home Health Registered Nurse Qualifications:
- Must have home health experience and would prefer them to have experience using HCHB, wound care/ wound vac, IV experience.
Licenses Required: Must be licensed and willing to accept compact license.
