Job DescriptionCase Manager (Temp) - Chicago, IL - $20-$25/hr.
We're seeking a Case Manager to support clients across Chicago, IL with coordinated services, benefits navigation, and resource referrals. Ideal for professionals who blend empathetic communication with organized follow through.
What You'll Do

  • Manage a client caseload; assess needs, create service plans, and track outcomes.
  • Coordinate with community partners, clinics, and providers; connect clients to appropriate resources.
  • Maintain accurate case notes and documentation; uphold confidentiality/HIPAA.
  • Educate clients on available services, benefits, and care options; advocate as needed.
  • Support program goals and reporting requirements; participate in team meetings and training.

What You'll Bring

  • 1-2+ years in case management, social services, care coordination, or patient navigation.
  • Strong interpersonal skills with cultural humility and crisis response awareness.
  • Organized, deadline oriented, and comfortable with EHR/CRM documentation.
  • Bilingual a plus (Spanish/English or other community languages).
