Fraser is seeking a dedicated Mental Health Adult Case Manager to support clients aged 18 and older. In this role, you will work directly with individuals and their families, collaborating with mental health professionals, primary care providers, educators, and community organizations. You’ll support clients from diverse backgrounds in achieving their personal goals and improving and maintaining their quality of life.
Requirements:
- Must have both:
- A bachelor’s degree or higher in one of the behavioral sciences or related fields from an accredited college or university.
- 2000 hours (1+ years) of supervised experience in the delivery of clinical services in the treatment of mental illness or adults/children with emotional disturbances.
- If holding a bachelor’s or master’s degree in Social Work, a valid LGSW license is required.

Responsibilities:

Conduct Functional Assessments for clients referred for case management.

Develop comprehensive Individual Family Community Support Plans (IFCSPs) or Individual Community Support Plans (ICSPs) in collaboration with the client and based on assessments.

Coordinate, monitor, and evaluate the effectiveness of services outlined in the support plan.

Communicate regularly with primary care providers, medical professionals, social services, and other providers to ensure integrated care.

Assist clients and their families in accessing services across mental health, education, health, social, and recreational domains.

Maintain a consistent caseload, scheduling and completing monthly case management meetings with each client/family

Location, Schedule & Pay:
- Fraser Bloomington/Virtual Office & Community-Based
- M-F, Full Time, Flexible Schedule - Regular in - person/ onsite work with clients is required.
- The starting pay for this role is $50,000 to $53,000 per year depending on qualifications.
