Position Summary:

The Fairbanks Rescue Mission is seeking a detail-oriented and organized SSVF Health Care Navigator to act as a part of a team at the Fairbanks Rescue Mission to provide assistance to Veterans and Veteran families in our community. The Health Care Navigator is essential to connecting disable Veterans with mental and medical facilities. The Health Care Navigator will work closely with the SSVF Case Manager to sure all Veterans have an opportunity to utilize these services.


Key Responsibilities:

  • Able to complete all required paperwork in a timely, accurate and professional manner.
  • Computer literacy, including knowledge of basic software applications and familiarity with the internet and email communications.
  • Meet with individual clients to review medical & mental health needs at regularly scheduled intervals.
  • Develop individual health care plans for qualifying participants.
  • Document all client contact and progress, including time and date, type of contact, outcome and plan of action.
  • Participate in the discharge process and aftercare needs of the client.
  • Provide regular updates to the Program Manager.

Qualifications:

  • Associate's degree in human services, related field or equivalent work history.
  • 2 years experience in delivering case management services.
  • Provide or broker services to holistically address clients needs in a timely and efficient manner to ensure goal accomplishment and procurement of health and/or mental health care services. These services include but are not limited to: assistance obtaining VA benefits; medical and mental health referrals; help scheduling appointments; transportation to and from appointments; reminder calls.
  • Excellent verbal, written and interpersonal communication skills.
  • Knowledge of veteran related issues and/or the homeless population with preference given to veterans.
  • Oversee coordination of services with other community service providers, including coordination of appointment times, facilitating activities and resolving issues.
  • Military Base access.
  • Valid Alaska drivers license.
  • Ability to work effectively with people of diverse educational and cultural backgrounds.


Work Environment:

  • Office-based position at the Fairbanks Rescue Mission.
  • Standard business hours, with occasional flexibility to meet program needs.
  • May involve occasional interaction with program participants or visitors.
