POSITION OVERVIEW
We have an opening for a full-time Residential Admissions Counselor opening at our primary Portland location. The Residential Admissions Counselor position provides professional assessment & initial treatment planning for substance use and co-occurring disorders. Duties and responsibilities include: performing intake assessments that utilize both ASAM and DSM5 criteria for diagnosis and placement at the appropriate level of care; collaboration with patients on the creation of an individualized treatment plan based on assessed needs, goals, and objectives; care coordination with treatment team members, community partners, and referents as needed; and provide group counseling.
ESSENTIAL JOB FUNCTIONS
Assessment and admission of patients
- Gather data systematically from the patient and other available collateral sources
- Complete individualized treatment plan in collaboration with patient
- Obtain and interpret all relevant assessment information
- Determine the patient's readiness for treatment and stage of change
- Describe the health and behavior problems related to substance use, including transmission and prevention of HIV/AIDS, TB, STDs, and other infectious diseases.
- Inform patient of their rights and responsibilities
- Document and reconcile patient medications based on current medication list.
- Review confidentiality guidelines
- Provide group counseling
- Review and follow up with referrals
- Complete screenings with potential patients
ADDITIONAL JOB RESPONSIBILITIES
- Attend All Staff meeting, and other meetings, as assigned.
- Communicate and coordinate with staff from other departments effectively.
- Maintain and strengthen good working relationships with clinical team members and management staff.
- Respond to inquiries about the residential program.
- Follow up on possible admits.
- Perform related tasks as assigned.
QUALIFICATIONS
- Current CADC R with the ability to obtain CADC I status within 6 months of hire
- Knowledge of Trauma Specific Services, Trauma Informed Care, Motivational Interviewing, DBT, and strong interdisciplinary team skills
- Proficient and timely with required productivity and clinical documentation standards
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
- Strong organizational skills and attention to detail
- Maintain confidentiality of sensitive information and documents
- Proficient in Microsoft Office applications
- Communicate information effectively in writing and verbally
- Understanding of SUD
OTHER REQUIREMENTS
- For employees with a history of substance use disorder or identifying as a person in recovery, Fora Health requires certification of a minimum of two years’ continuous sobriety.
- Successful completion of pre-employment drug test
- Successful completion of Tuberculin test and/or evaluation with negative results or documented evidence of non-communicability
- DHS criminal background approval
SUPERVISOR FUNCTIONS
This position has no supervisory duties.
WORKING CONDITIONS
- Prolonged use of computer and working at a desk
- Position generally works in an indoor office environment with occasional travel between sites or to special events.
- Possible exposure to communicable diseases which can encompass a variety of illnesses and infections, including COVID, tuberculosis, Hepatitis A, B, and C, and HIV.
- Possible exposure to upset, angry, traumatized, disabled or emotionally disturbed adults, adolescents, and families.
- Potential exposure to potentially hazardous cleaning chemicals, personal protective equipment provided.
HOURS AND PAY
This is a full time position earning $26.68-29.48 per hour (depending upon experience). This position is non-exempt and eligible for overtime compensation.
LOCATION
This position is an on-site position at our SE Cherry Blossom location in Portland, Oregon.
