Your Role:
- Deliver skilled nursing services in-home, including wound care, medication administration, diabetes care, post-surgical care, injections, and more
- Work 1:1 with patients, building trust and rapport over time
- Collaborate closely with our care team and the patient's physician
- Maintain clear, accurate documentation and comply with DOH standards
Who You Are:
- Licensed Practical Nurse (LPN) licensed in Pennsylvania
- CPR-certified and able to travel locally with your own vehicle
- A skilled communicator with a kind, patient-centered approach
- Experience in home health or long-term care is a plus, but not required
- Someone who values relationships, teamwork, and compassionate care
