We are seeking a Home Health Physical Therapist to join our team! You will plan physical rehabilitative programs to improve patient quality of life. Muller &amp; Weber Physical Therapy is a therapist owned private outpatient practice. We have been serving the Santa Barbara and its surrounding areas for over 40 years. We treat a wide range of orthopedic conditions. In addition to the services already mentioned we also provide Home Health therapy providing patients smooth transition from hospital, home and then to office setting. The ideal candidate is a skilled professional that is passionate about delivering quality care and making a real difference.&nbsp;
Responsibilities
<ul>
<li>Assess each patient and develop an appropriate treatment plan with measurable goals and time frames</li>
<li>Prepare and submit clinical and progress summaries based on identified and attained goals</li>
<li>Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient</li>
</ul>
 
Qualifications
<ul>
<li>Bachelor's, Master's or Doctor's degree of an accredited physical therapy curriculum</li>
<li>Certification in Physical Therapy and valid state license to practice</li>
