To support our growth and the increasing volume of client care in Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Recertification Nurse to join our agency and serve existing and returning clients throughout southeastern Ohio. This position requires travel to several counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or travel).

Exact service areas are flexible, with common locations including:

  • Adams, Brown, Highland, Clinton, and Fayette Counties
  • Jackson, Vinton, Pike, Scioto, and Gallia Counties
  • Highland, Adams, Fayette, Ross, and Pike Counties
  • Athens, Hocking, Perry, Vinton, and Washington Counties
  • Coshocton, Fairfield, Knox, Licking, and Muskingum Counties

This is a long-term employment opportunity that can be built for one Full-time (preferred) or multiple Part-time nurses to fit your schedule - whether that's:

  • as a 2nd job (working around your primary schedule)
  • as a consistent Part-Time position
  • starting at Part-Time and increasing to Full-Time in the first 30-60 days
  • as a Full-Time, primary employment position (requires occasional travel up to 2 hours from home)
  • as a Full-Time position with a defined path for advancement within 1-2 years (to RNCM, Assistant Program Administrator, Nursing Supervisor, or other)
  • as a PT or FT position for an experienced Case Manager looking to step back into or out of the industry/workplace with reduced headaches
  • any self-motivated home health RN looking to gain on-the-job Case Management training and experience and/or work in a consistent, professional role with autonomy, flexibility, and work-life balance!

  • Pay is weekly, either at an hourly rate or negotiated salary (with minimum # recerts required per week).
  • Typical travel distances include a 60-90-minute radius daily (or less) + up to 2 hours on occasion, with the option for Overtime as available (not required).
  • Opportunities for advancement into Administrative, QA & Compliance, and other leadership positions are available over time (though not required).

  • Pay is hourly, every week.
  • Typical travel distances include up to a 1-hour radius daily (or less) + additional hours for further distances as willing / available.
  • Opportunities to evolve into a full-time position is available after 90 days if desired (not required).


More about the role:

  • Exact travel locations and daily shifts are customizable based on your availability and willingness to travel.
    • Typical hours are between Monday - Friday (weekdays) during the day (8am - 5pm).
    • Round-trip / Door-to-Door mileage reimbursement provided.
  • Typical visits take place during a standard 8am - 5pm weekday schedule, however many clients attend school and cannot be seen until 3-5pm, so this role requires visits occasionally outside of this schedule.
  • Services include starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as new hire orientations and supervisory reviews. *Services may also require a few weekly visits.
  • At least 2 years of experience in home health is required.
    • Must have experience with OASIS and NON-OASIS assessments.

Compensation:

  • Hourly: starting at $35/hr
  • Annual Salary (+ PTO): negotiable based on agreed-upon minimum recertifications per week


Job Summary
As an RN Recertification Nurse, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.

  • With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Reports to: Regional Program Administrator, or Clinical Administrator/Director of Nursing


Qualifications

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of 2 years of experience in home health agency or equivalent setting.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience with OASIS and NON-OASIS assessments is also required.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
  • Willingness to travel to clients in multiple counties.
